The manufacturer decided its own vehicle has a safety defect and told the department. This is the ordinary case and by far the largest group.
It matters that the maker filed it rather than the regulator: the law in most countries requires a manufacturer to report a defect once it knows of one, and the date on the notice is the date it did.
