The driver�s seat belt might have the webbing routed through the anchor rubber trim sleeve instead of being routed through the metal anchor eyelet.

Remedy, as DVSA words it

Carry out a visual inspection of the driver seat belt at the pre-tensioner. Any vehicle that has failed this inspection will be retained and the affected part will be replaced.
